The SBA has revised the the available Forgiveness Applications: the 3508S, 3508EZ, 3508 Standard forms. Each form and instructions are available for download below. 

PPP Loan Forgiveness Applications and Instructions

Specific to the 3508S, this is a document that has been revised and is specific for loans <$150,000. We are encouraging our borrowers whom fall within this category to utilize this form and work to expedite their forgiveness.

For borrowers whom are applying or have yet to apply for forgiveness for Round 1.0, this document allows for the borrower to complete and provide all attestations/certifications in a simplified form without the requirement to produce supporting documentation at the time of submission.

The 3508S does not require borrowers to show their calculations used to determine their loan forgiveness amount. However, the SBA may request information and documents to review those calculations as part of its loan review or audit process.
